Has anyone run any D&D/AD&D campaign settings using Pathfinder?

I have considered running a Pathfinder game using Planescape or Eberron. Has anyone achieved such a mismash successfully? Were there any painful moments converting anything in particular to the new rules? Was it possible to port it straight over with no conversion?

Yes to all except "Were there any painful moments converting anything in particular to the new rules?" and resounding YES to "Was it possible to port it straight over with no conversion?".

Start with free conversion guide to nail the most important differences. The worst that can happen, is that you use an older version of a rule.
It's not a problem as long as you make it clear to players that such things are possible.

Secondly, enjoy the game.

That's all... well, you may want to point out to your players that they have more options to choose from now.
