Sweet. Love the grayed-out icons. A few things I would tweak:

The init box, speed box, and passive skill boxes don't need that much space. You're only going to write down +x, so these can be shortened.

The defense boxes could be shortened, if you wrote conditional modifiers in the notes. Not really necessary.

The Skills could use more space, mostly for mods that apply only in certain situations, like mundane item bonuses (climbers kit/lock picks), short duration buffs, etc.

One of my major peeves is note sections that aren't ruled. I have a bad tendency to start angling my sentences down if I don't have a line to measure against, so I'd line that out if it were me.

Other than that, top notch. If you make some of those changes, I'll use these for sure. Thanks again! :)

Your right, but honestly didnt have something else to put there, so I left them as is. Any suggestions welcome.

I have about 1/2 an inch to play with in hieght so I can increase the skill size to a more managable size.

I can add faint ruler lines.

Here's one I use

A template for creatures, then one for characters. The bottom of the latter cards has a list of the harder-to-remember conditions, which could be removed in favor of notes later. I let the players keep track of surges and APs, but they could easily be added to the cards.

Highlighted Stealth and Perception in red, to help find them quicker. Just add 10 for passive.

To be true initiative cards, you must have a box in the upper left of the card for current initiative. I used the cards from the DMG last week, and in the middle of the combat, dropped the cards - having a place where I penciled in the current initiative allowed me to recover quickly from this.

Your initiative cards are a little too much like mini character sheets. Having the passive skills listed is good, but I don't think the DM should be rolling things like Endurance saves vs disease and similar things which don't have passive saves. Unless you don't trust your players, this is not information (e.g. Athletic skill for jump rolls) that the DM needs to track in a card that is primarily used for initiative.
