This is a Leophius (also from Necropolis), advanced to 19 HD and Huge size, with the Corrupted template (Book of Vile Darkness).

Constrict (Ex): The Crawling Horror crushes the opponent, dealing 3d6+7 points of bludgeoning damage, after making a successful grapple check

Disruptive Attack (Su):
The Crawling Horror deals 9 additional points of vile damage when it touches uncorrupted, living, corporeal nonoutsiders. Vile damage can only be healed by magic cast within the area of a consecrate or hallow spell.

Improved Grab (Ex): To use this ability, the Crawling Horror must hit with her bite attack. If she gets a hold, she can constrict.

Scent (Ex): The Crawling Horror can detect approaching enemies, sniff out hidden foes, and track by sense of smell.

Physical Description: A bloated, twisted snake with the head of a lion.

OK, this one's a little nutty. Took a dire toad (MMII), added the Monstrous template (Savage Species), advanced her out to 10 HD, added the Thessalmonster template (Tome of Horrors), and then added the Monster of Legend template (MMII).

Note that technically, adding the Thessalmonster template would have changed the monster type to Abberation, which should prevent the addition of the Monster of Legend template. Sometimes you just gotta forget what the book says.

Breath Weapon (Su): 30-foot cone, once every 2d4 rounds, 2d8 cold, Reflex DC 31 half. The save DC is Constitution-based.

Improved Grab (Ex): If the Venomous Beast hits an opponent that is at least Large size with a bite attack from its toad head or a tongue attack, it deals normal damage and attempts to start a grapple as a free action without provoking an attack of opportunity. If it gets a hold, it can try to swallow an opponent. Alternatively, the Venomous Beast has the option to conduct the grapple normally, or simply use its jaws to hold the opponent (-20 penalty on grapple check, but the Venomous Beast is not considered grappled). In either case, each successful grapple check it makes during successive rounds automatically deals bite damage.

Poison (Ex): Injury, DC 31 Fortitude save, initial and secondary damage 1d6 Con. The save DC is Constitution-based.

Spells: The Venomous Beast casts divine spells from the cleric list and from the Protection, Strength, and War domains as a 5th-level cleric. It does not gain extra domain spell slots.

Spells Prepared (5/5/3/2, DC 15 + spell level): 0—guidance, inflict minor wounds (2), resistance, virtue; 1—bane, cause fear, doom, obscuring mist, shield of faith; 2—bull's strength, hold person, silence; 3—bestow curse, deeper darkness.

Spit Acid (Ex): The Venomous Beast’s snakes can collectively spit a stream of acid 5 feet high, 5 feet wide, and 40 long once per hour; damage 4d6 acid, Reflex save for half. The save DC is Constitution based.

Swallow Whole (Ex): The Venomous Beast can use its toad head to swallow a grabbed opponent that is at least Large size by making a successful grapple check. Once inside the Venomous Beast, the opponent takes 2d6 points of bludgeoning damage plus 1d8 points of acid damage per round from the Venomous Beast’s stomach. A successful grapple check allows the swallowed creature to climb out of the stomach and return to the Venomous Beast’s mouth, where another successful grapple check is needed to get free. Alternatively, a swallowed creature can try to cut its way out with either claws or a light piercing or slashing weapon. Dealing at least 10 points of damage to the gizzard (AC 18) in this way creates an opening large enough to permit escape. Once a single swallowed creature exits, muscular action closes the hole; thus, another swallowed opponent must cut its own way out.

Blindsight (Ex): The Venomous Beast can ascertain prey by scent and vibration within 60 feet. Invisibility and darkness are irrelevant, though the Beast still can't discern ethereal beings. The Venomous Beast usually does not need to make Spot or Listen checks to notice creatures within range of its blindsight ability.

Cold Subtype: The Venomous Beast is immune to cold damage. It takes half again as much (+50%) damage as normal from fire, regardless of whether a saving throw is allowed, or if the save is a success or failure.

Physical Description: The Venomous Beast has a squat, draconian body, crowned by a huge and bloated toad head ringed with eight, writhing snakes.
