Here's some relevant information I found in the SRD (3.5)

Exhausted: An exhausted character moves at half speed and takes a –6 penalty to Strength and Dexterity. After 1 hour of complete rest, an exhausted character becomes fatigued. A fatigued character becomes exhausted by doing something else that would normally cause fatigue.
Fatigued: A fatigued character can neither run nor charge and takes a –2 penalty to Strength and Dexterity. Doing anything that would normally cause fatigue causes the fatigued character to become exhausted. After 8 hours of complete rest, fatigued characters are no longer fatigued.

STARVATION AND THIRST
Characters might find themselves without food or water and with no means to obtain them. In normal climates, Medium characters need at least a gallon of fluids and about a pound of decent food per day to avoid starvation. (Small characters need half as much.) In very hot climates, characters need two or three times as much water to avoid dehydration.
A character can go without water for 1 day plus a number of hours equal to his Constitution score. After this time, the character must make a Constitution check each hour (DC 10, +1 for each previous check) or take 1d6 points of nonlethal damage.
A character can go without food for 3 days, in growing discomfort. After this time, the character must make a Constitution check each day (DC 10, +1 for each previous check) or take 1d6 points of nonlethal damage.
Characters who have taken nonlethal damage from lack of food or water are fatigued. Nonlethal damage from thirst or starvation cannot be recovered until the character gets food or water, as needed—not even magic that restores hit points heals this damage.

Forced March: In a day of normal walking, a character walks for 8 hours. The rest of the daylight time is spent making and breaking camp, resting, and eating.
A character can walk for more than 8 hours in a day by making a forced march. For each hour of marching beyond 8 hours, a Constitution check (DC 10, +2 per extra hour) is required. If the check fails, the character takes 1d6 points of nonlethal damage. A character who takes any nonlethal damage from a forced march becomes fatigued. Eliminating the nonlethal damage also eliminates the fatigue. It’s possible for a character to march into unconsciousness by pushing himself too hard.

Sleeping in Armor: If a character sleeps in a suit of armor with an armor check penalty of -5 or worse, the character is automatically fatigued the next day. The character suffers a -2 penalty on Strength and Dexterity, and can't charge or run.

Basically, I think you have to make Constitution checks in order NOT to take non-lethal damage. If you do take non-lethal damage, you are fatigued until you can rest (8 hours)...

I'd run it this way:

Lack of sleep: A character can go without sleep for 1 day plus a number of hours equal to his Constitution score. After this time, the character must make a Constitution check each hour (DC 10, +1 for each previous check) or take 1d6 points of nonlethal damage.

A character who takes nonlethal damage from lack of sleep if fatigued. Further sleep-deprivation nonlethal damage would make that fatigued character exhausted.

Very much like having no water. It works for me. Rule-based and very simple to implement.

That is because there are not any rules on penalties accrued due to lack of sleep. If you fail to sleep you fail to gain the benefits of sleep.

I think you can rule that the "day" in the Forced March rule does not reset until you get 8 hours of sleep. If a day in the Forced March entry is interpreted as the time between sleep cycles this creates a serious barrier to staying awake for long periods of time. Especial if you count other strenuous actives against this time limit.
 

Seems kind of dumb that they wouldn't have made any rules for it... I have PC's that have gone 36 hours without sleep... and there are no rules for this? Ugh :(
Oh well. Thanks for the help guys :), I'll try to convert the existing rules on forced march, etc...
