So do I have to hold this pearl.. or what?

The DMG entry for the Pearl of the Sirines says:

"if it clasped firmly in hand or held to the breast while the possessor attempts actions related to the pearl's powers, she understands and is able to employ the item."

So, does that mean in order to use the pearl, you have to always hold it in one hand? Which of course means you couldn't use that hand for other things (spellcasting, weapon holding, etc..)

Or does it just mean that's how you know what the pearl does? So, once you know it for what it is, you can just hold it in your pocket while you swim freely?

Am I the only one who thinks this is confusing? Is it clear to everyone else? If so, lemme know what I am missing.

I wouldn't say ALWAYS. Just always when you want to use it. So, basically, under water.
 

I'd interpret "held to the breast" to allow wearing it on a neck-chain against your skin, so you choose whether it takes up one hand or the amulet slot. If it's just in your belt pouch it provides no assistance.
