Quick question, do ability score increasing magic items stack, such as a headband of Intellect (+2) and an iode stone (+2 int) for a total of + four?

No. Both are Enhancement bonuses, and with a very small handful of exceptions, bonuses of like type don't stack.

From the SRD:

Stacking Effects: Spells that provide bonuses or penalties on attack rolls, damage rolls, saving throws, and other attributes usually do not stack with themselves.

More generally, two bonuses of the same type don’t stack even if they come from different spells (or from effects other than spells; see Bonus Types, above).

However, the enhancement bonus from a headband, ioun stone, or fox's cunning spell would stack with the inherent bonus from a tome of clear thought or wish spell, or some other bonus of a different type.

Here's another one
if I wanted to make the sword a “keen” defender +X greatsword do I have to pay the fee for a +1 enhancement (which is what the keen ability is equivalent to) or do I have to pay the difference between a +4 (which is what a +3 defender is equivalent to) and a +5? The first way is a 2,000 gp cost, while the second is a 18,000 gp cost

A +3 defending Greatsword, has a market value of 18,350gp. The market value of adding keen to that weapon, would be 14,000gp.

The Defending property is worth a further +1 enhancement; so a +3 defending greatsword would cost 32,350GP.

Upgrading it to a +3 keen defending greatsword costs the difference between the cost of a +4 weapon and a +5 weapon; hence 18,000 GP as you say.
