Successful guardians

Do guardians (alert soldiers, set golems, bound demons, etc.) ever actually succeed in their job against adventurers? Do you have any tales of times when adventurers were stopped, deflected, repelled, killed, etc. by any kind of "guardian".

Does a dragon who trapped and guarded his lair count? If so, he beat the same PC who tried to attack him multiple times.

There was also a big temple guarded by a Prismatic Titan named Themis with seven levels in Initiate of the Sevenfold Veil, and she beat up most of the PCs and could have killed them, but she figured out that they were the people who were supposed to be there and let them in.

Also, there was an undead guarding some ruins who hit one of the PCs so hard in the first round that he ran away as fast as he could and convinced the other PCs never to enter the ruins again--ever (and he was level 19 at the time).

I fairly routinely have dungeons that the first part has been explored, then the PCs will slowly get to areas that contain the origional guardians. Traps will be triggered inside, bones of fallen adventurers who didn't make it too far. Many times when I do this, I'll have some kind of injured construct/undead (who don't heal). So, the PCs could run into a clay golem with 1/3 hp, cracks in its body, and dead skeletons laying around of the adventurers who almost had it killed, but died valliantly.

Often, the PCs will meet up with lone survivers of a failed adventurer group. Of course, right now they're in a city with a high % of adventurers. Their group died, and they might give up on the life and sell the location, in the case of some good NPCs simply with the request to bury their fallen comrades or even bring them back for ressurection. Other times, they'll want to accompany the PCs to the dungeon for a second try and an equal share of the loot.

Then there are the good guardians who the PCs don't want to kill and so leave alone whatever they are guarding. Sometimes they're killed by other groups, but not usually, and so far when it has happened the PCs have never returned to find out about it anyway. Once they met up with a celestial who had failed in his guardianship but lived. I had him join the PC group after that seeking redemption. I didn't understand the whole ECL thing back then, though... he was a bit too powerful, and so that didn't work out long. Oops.
 

In the last campaign that I ran the PC's battled into the very heart of the enemy stronghold as they desperately tried to stop them from completing a dark ritual. They were turned away short of the inner sanctum by a Forbiddance and failed to stop the evil plot.

Does that count?
