Special Attacks: A half-fiend retains all the special attacks of the base creature. Half-fiends with an Intelligence or Wisdom score of 8 or higher possess the following spell-like abilities, using their level as the caster level, as specified in the table below.

If I add this template to a monster with no class levels. Does the monsters HD count as monster levels? Giving the monster access to the spell like abilitys.

Yes. The MM's example shows that a Half-Fiend/Half-Medusa (6HD) can use Half-Fiend abilities up to 6th level.

Your right. I misted that.
But how is the save DC calculated?
Like from an item or like if the monster is the caster adding any ability modifiers?

A Half-Fiend Half-Medusa is an undefined base creature with two templates.

DMG p.94, Difficulty Classes. DC = 10 + Spell Level + Modifier. In this case Wis, because they are spell-like abilities emulating divine spells.

minor nitpick. Its a Half-Fiend/Half-Medusa. At least according to page 216 of the Monster Manual.

A Half-Fiend/Half-Medusa is a Medusa with the Half-Fiend template.
