In the "Labyrinth of Madness" it describes creatures that are under the control of the Lifebane entity as ex. Lifebane Beholder. It also states that changes made to creatures or PC's do not go away if they ever escape. That seems like a template to me.
Here is my thinking so far. The Spellwarped template in the MM3 is close, but needs some additions and subtractions.
Lifebane Template
Adding the Lifebane Creature Template
“Lifebane” is a template that can be added to any creature (referred to hereafter as the base creature). The Lifebane template can only be gained by entering the ancestral home of the dyris in the Labyrinth of Madness. Creatures affected are corrupted with physical changes as well as mental. Physical changes include, but are not limited to: scaly skin, serpentine features, snake tail, and even snake heads protruding from the body.
Size and Type: The base creature’s type does not change, but the creature gains the Lifebane subtype.
A Lifebane creature uses either the base creature’s statistics and special abilities in addition to those described here.
Hit Dice and Hit Points: Same as the base creature.
Speed: Same as the base creature.
Armor Class: The base creature’s natural armor bonus improves by +2.
Base Attack/Grapple: Same as the base creature.
Attacks: Same as the base creature.
Damage: Same as the base creature.
Special Attacks: A Lifebane retains the special attacks of the base creature.
Need something here about physical alterations such as scales, snake heads, tails like fiendish template.
Special Qualities: A Lifebane retains all the special qualities of the base, and also gains those described below.
Corrupted Madness (Su): A Lifebane affected creatures alignment becomes chaotic evil and the otherworldy energies that are the Lifebane can shift a creatures mind and spirit, forever twisting the person until she is horribly evil.
Spell Resistance (Ex): An afflicted Lifebane creature has spell resistance 11+ its Hit Dice. If the creature already had spell resistance use the greater of the two values.
Lifebane Sense (Ex): Any creature with the Lifebane template may locate other creatures with the Lifebane template at will.
Base Save Bonuses: Same as the base creature.
Abilities: Same as the base creature.
Skills: Same as the base creature.
Feats: Same as the base creature.
Environment: Labyrinth of Madness
Organization: Unique
Challenge Rating: By class level or base creature.
Treasure: As per creature.
Alignment: Chaotic Evil
Advancement: By character class.
Level Adjustment: Same as the base creature +1.
