Merric mentioned EX1 and EX2 in another thread, and I think they're very appropriate herein as well, in particular because they lull you with their recreation of "nonsensical children's story time" when in fact they harbor very deadly encounters. Which is, of course, part of their fun :D

N.B.: EX1 is the only AD&D module published by TSR (outside of Dragon's Fedifensor) in which I recall a vorpal weapon appearing (it is used against the PCs, of course!).
 

Night of the Seven Swords, an old 1stEd Oriental Adventure.

For the supposed character level (5 - 7 IIRC) it was a killer. Mostly really nasty foes but some dangerous traps as well. Boss monster turns out to be a 50HD gargantua. Then, when the PC's return with the 7 swords they find their contact has been killed and replaced by ninja. (damn ninja).

All up from a party of 7 (all well played bar one who died in the first encounter, while running away) I had 2 survivors. ONe guy with just enough HP to stand and another who was polymorphed into a sparrow with only 1 good wing (couldn't even fly.)

What I like about it over say, ToH, is that the players knew when danger was coming. It wasn't just a case of 'I'll stick my head in this dark whole' and die. They very quickly worked out they were in trouble and worked well together to face it.

So in answer to the question (I knew I'd get there eventually) ToH and similar: nah. No fun for anyone but the GM. ANd even then they'd have to be a sick <expletive>.

I recently ran the 3.5 update from wizards.com of White Plume Mountain for my players. I had to beef up the encounters because they were in the 10 to 11 level range and the revised version was set at around level 7. They had a very hard time with it. There were I believe 3 character deaths and 1 tpk. I'm pretty sure I messed up and made it too tuff. I don't know if it was supposed to be a killer dungeon but it turned into one. The players didn't complain though. They actually loved the sense of danger.

I've played in Rapan Athuk and it is most assuredly an awesome killer!
The 2e Labarynth of Madness from 2e was a slaughterhouse.
The Ruins of Myth Drannor was an absolute bloodbath when we played it back in the day.
