Labyrinth of Madness, as mentioned by several people. Note that it can be played in several "modes", depending exactly on how badly the DM wants to annoy the players.

Mordenkainen's Fantastic Adventure. Not really a killer dungeon, but noted for a few TPK inducing encounters.

how many of you have specifically designed a scenario/adventure/campaign to be a particularly deady situation?

we sometimes (not often) do this to test characters/players.

just curious.

we tend to run things on the higher end of the danger spectrum, but sometimes there is no substitute for "over the top".

there are rewards for performing well.

these situations exist outside the game so deaths aren't real. we don't tell our players that before hand. happy to say, the players/characters almost invariably aquit themselves well.
