1. Make the prison 'sentient'. Have it evolve itself to combat attempted escapes. This could be an 'undermountain' effect in that the 'floorplan' changes - suddenly the exit has become the entrance to 'C-Block'... Door automatically open/close, lock/unlock - unwittingly leading the escapees back to their cells... etc, etc. It could even come with it's own automated defences.
2. Make it self governed. There was a PC game - Gothic, I think - whos story was that ill-doers get thrown through a 1-way portal to a prison plane/planet. No guards, no cells, no nothing. Just criminals - whom being evil provided all the punishment themselves!
3. Make it a 'alien'. Change everything from the normal campaign on it's head. That should provide both enough interest & confusion to your players.
4. Soul bond - This is 'hijacked' from a game I'm currently playing on these forums. Upon entrance to the prison (us PC's - 6 of us) have had our souls bound to another inmate. The group has LG & CE characters, yet they work together because they know that killing one of us would also kill another - whom, we don't know!
5. Have the only prison exit lead directly to the lowest levels of the abyss... Do not pass GO and do not collect $200...

Simple, yet effective. Unfortunately, it wouldn't work in this prison for reasons I can't divulge as my sneaky players--even though they were told to stay out of this thread--are probably reading this right now.Slife said:

Combining Roger's collar idea with the a'dam from the Wheel of Time series, how about every inmate wears a collar that's designed to cause pain under a number of conditions, these conditions are things like:
How exactly the pain is handled in terms of game mechanics is up to you. It could cause damage or simply an incapacitating condition, like nauseated.
The weak point in this system would be a non-collared person taking a collar off an inmate, but that's what the guards are for. Your "unknown factor" could be a person who has snuck in and looks like he's wearing a collar, but it's really an ordinary non-magical collar. Detecting such a person would be difficult, since you'd have to do it without using magic (or you'd cause a lot of undue pain in nearby inmates, and the magic would just be absorbed by the collars anyway).
Edit: (I keep thinking up more things related to this...) And inmates who have been there long enough would probably have been "trained" to the point where they don't want to escape. Somebody would end up dragging them to the door, telling them their collar is gone and they won't feel any pain, but they'll be so mentally damaged they won't believe and will be shrieking for a guard to come "rescue" them.
- The prisoner touching his collar or any other collar (being normally worn does not count as touching).
- The prisoner thinking of escaping or having thoughts of violence against the guards.
- The prisoner leaving designated areas of the prison.
- A nearby guard experiencing pain (a guard in pain causes 10 times as much pain in nearby inmates wearing collars).
- Magic of any sort being cast in the vicinity of the collar or targeted at the collar or its wearer. (In addition, such magic is absorbed by the collar and does not actually produce its intended effect.)
- Pain can also be induced at will by the guards for any reason
