Not knowing much about the series myself, I'd say the most natural fit would be the Ranger, Hunter archetype. Of course, Geralt's unique abilities may require a custom race, and be potentially unbalancing if modeled correctly.
 

Proficiency with Alchemy toolkit - regardless of class.
Bear Totem Barbarian 4/(Champion or Battlemaster) Fighter 12/Hunter Ranger 4 for more alchemy/swordplay focused Gerald.
Favored Soul Sorcerer 12/(Champion or Battlemaster) Fighter 4/Warlock 4 if you want more magic/swordplay focused Gerald.

Pure Valor Bard or Eldrtich Knight Fighter are also attractive options as standalone classes.
 

A Witcher racial variant might have a bonus (+2) to Strength and Dexterity, +1 on HD rolls, Darkvision, and immunity to poison.

I think going at least 7 levels here for the 4 spells, Evasion and Stillness of Mind. 10th level Purity of Body and possibly 14th level Diamond Soul are all great IMO.

Aard - Fist of Unbroken Air + Stunning Strike (if required)
Igni - Sweeping Cinder Strike, Fangs of the Fire Snake, Hatchling’s Flame, Red Dragon’s Claws all possible.
Quen - Deflect Missiles+Impenetrable Iron Tortoise Shell, Become the Teapot
Axii - ~ Possibly Hold Person
Yrden - ~

Other interesting spells: Unrelenting Flames, Eyes of Fire
Continuing to level 11 opens up other possibilities.

As a monk you can wield staff, call it whatever, like a Witcher Blade, and do 1d8 finesse while still attacking with your Martial Arts as a bonus action.

Elf, Wood Level 10, Monk of the Four Elements
10, 20, 14, 10, 14, 12 - Depending on how powergame-y you go, change this.

Martial Arts, Unarmored Movement, Deflect Missiles, Slow Fall, Stunning Strike, Evasion, Stillness of Mind, Purity of Body - all thematic

3: Fist of Unbroken Air, Red Dragons Claws
6: Impenetrable Iron Tortoise Shell, Eyes of Fire

Good question.

I would vote for a warlock - they make a pact with the ideal of being witcher, rather than patron. I think you could make it work. Maybe a fighter multiclass.

One thing about witchers that comes through in the books but not in the computer games is that Geralt is a particularly powerful witcher who who undertook unusual mutations. He is not a first level bog standard Witcher! Furthermore all the witchers left are old ones, so there is some space for first level witchers to be weaker than portrayed in the computer games IMO.
 

Fighter (Eldritch Knight) definitely.
Without house rules and the potential for imbalance, this is the closest class to a Witcher. Lots of good swordfight and a few useful spells.

For race I'd use Half-Orc because of the stats, darkvision and the endurance. Obviously not looking like an Orc.
Otherwise Human is fine.

Edit: Plus a background that gives you proficiency with Alchemist kit or Herbalism kit. I think Hermit is a good one, for example.

I'd go with Eldritch Knight as well. His magic isn't anywhere near as powerful as a Warlock. He can't pick locks. His survival skills are good but that can taken as background. I would be hard pressed to give him low stats in anything. The guy is better than other people in almost every way.
