I noticed that the preview has at least one conversion (Giant Pike, the rest look new as far as I can tell), but that ToH2 doesn't follow the same Section 15 set-up that ToH does... Any particular reason/s for the more standard approach?

Yep- giant pike isnt a conversion. Yes, it appeared in 1e AD&D (so did the Cave Lion which is also in the book), but they are not conversions per se (just happen to be same type of monsters as in a previous edition of the game). So, therefore we just went with "standard" Section 15 stuff (and everything, except a few specific place names and the artwork of course, is completely OGC).
 

wait a minute, no conversions? i thought the title was converted from the original Tome of Horrors, with a "II" added. ;)

Ok- one conversion..the title. :) Actually, two. The Piercer appears in its 3.5 incarnation (updated from Tome 1) as a sidebar for the Crag Man entry (they can summon a piercer storm to "hinder" their foes.)
