Has nobody mentioned something from Open Design yet? I haven't been able to patronize any yet :(, but the unusual business model should make Baur's stuff pretty important.
 

I aggree with pretty much everything already mentioned as being good to excellent.

I would like to add Dave Arneson's Blackmoor setting to the list. The setting has some cool stuff: new base classes, some racial variants, spell focus crystals. The Wizard's Cabal book expands on the history of the setting and adds to the magic variants for the setting. The Dungeons of Castle Blackmoor adventure is a straight up dungeon crawl that a party could easily play from 1st to 20th level.
The setting understandably has an old-school feel to it. I dig it. I f you like the old-school feel D&D you may dig it too.
