Two-Weapon Fighting/BAB Question - HELP!

Alright, I'm totally weirded out trying to figure out full attacks for this NPC I'm whipping up for a game, so I need some forum help before I pull what little hair I have left out of my skull.....

He's a 4th level Fighter, 2nd level Barbarian and a 7th level Blessed of Gruumsh (from Dragon Magazine), which gives him a total BAB of +13.

He has a strength of 21, which gives him a +5 to hit.

He has weapon focus (orc double axe) whioch gives him another +1 to hit with that weapon.

He has both the two-weapon fighting and improved two-weapon fighting feats.

What I need to know is what are the numbers for a full attack made by this NPC, including the penalties for using his orc double axe. I figure that he has five total attacks, but I'm not sure.

HELP!!!!!

BAB is +13/+8/+3. His other bonuses make that +19/+14/+9. Two-weapon fighting gives him another attack at his primary BAB, and improved TWF yet another at his secondary attack bonus, with -2 to all attacks (double weapon penalties are as for a one-handed and a light weapon pair), so the net totals are +17/+17/+12/+12/+7 . Note that one +17 attack and one +12 attack are off-hand attacks and will only get a +2 Strength bonus to damage instead of +5 ...
