Two-Weapon Fighting

I'm probably just being stupid.. but, when I am wielding two weapons, I know I get one extra attack per round.. but what about at later levels, when I can get more than one attack per round anyways (on full attack)..

example:

Level 6 fighter (with Ambidex. and TWF, and a light weapon, assume strength 10)

Is it:
Attacks: +4/+4(offhand)/+1

or:

Attacks: +4/+4(offhand)/-1/-1(offhand)

Neither.

+4 (first attack) / +4 (off-hand) / -1 (second attack)


Breakdown:
1) +6 -2 = +4
2) +6 -2 = +4
3) +6 -5 -2 = -1
