Ask your DM to consider a house rule for hit points, that you always get at least half the HD in case you roll lower.

Then you can stand in the back of the party while others fight in melee.

Sadly, the way to attain ultimate AC as a melee character is to build a melee artificer. I'm not really sure what the highest AC one can hit as a fighter is, but my general rule of thumb for AC optimization is to stop once you get 5 * character level (obviously you'll exceed this easily at levels below 8 or so.)

The only things I'd add are - Dwarf (+4 AC vs giants), tower shield and Mobility...
