What level is the group? there is a good chance a high level group has made a name for themselves and are subject to knowlege: local rolls.

If they and by extension their items are known about, an opening Sunder {disarm if you are feeling generous] attack is a good idea for really strong opponents.

Option one, don't worry about girth, but the fighter needs to use a readied action for when the creature attacks with it's mouth.

Option two, alter what Vorpal does. Extra damage, automatic Coup d' Grace on a 20, allow the fighter to roll confirmations on the crit until he misses, and add the multiple crits in one hit, etc.

Or, if you don't feel like changing anything, anything with a neck dies on a 20.
