One last question - Earthquake (the spell)can be used to collapse a ceiling and creatures can get pinned. How do you get out?

Others have hit the wall question pretty thoroughly; I just thought I'd chime in on the earthquake issue.

I believe I'd use the rules in the dmg for cave-ins. Hm, you might need help to escape- I'm not certain.
