The way I read it, the wizard teleports, as does a willing creature within 5 feet of the manifest mind. The mind doesn't teleport, nor does its size factor into the size of creature that can be teleported, but the distance of the journey is measured from the manifest mind's space.

Yup. The mind cannot itself move with Dimension Door (unless it is being carried). It is not the caster of the spell, nor is it a creature.
