Heroquest. It was first announced in the RuneQuest rulebook in 1978, and released in 2003, 25 years later.
There were several versions being playtested during the mean time, none of which bore much resemblance to the final product (which on the other hand besically was a second edition of Hero Wars*).
* Hero Wars wasn't called HeroQuest, because in the years between RQ and HQ the name HeroQuest was hijacked by Games Workshop, who, by the way, earlier had been the UK printer and distributor of RuneQuest.
