In 3.0, when keen and Improved Critical stacked, you could do the following.

Take an iron/steel weapon with a threat range of 18-20 (such as a scimitar)
Put keen on it - threat range increases to 15-20.
Improved Critical - threat range increases to 12-20.
Take 4 levels in Disciple of Dispater (Book of Vile Darkness) and gain the iron power ability
Final critical threat range - 9 - 20!
 

You know... none of that is terribly Munchkiny (Well, except for the 3.0 crit rules... but those were fixed for a reason). The spell theif combo isn't possible until level 15, and a total bonus of +3 for having any exotic weapon isn't really worth it.

Anyways, those things are more along the lines of powergaming, not munchkining. Munchkins cheat and/or bend the rules, powergamers make the most of the rules presented to them. Also, shouldn't rules discussions be in the appropriate forum?
 

My best was in 3.0:

scythe wielding-character (2d4x4)
ranger level with favored enemy: undead
Supernatural Blow feat - you don't crit undead, but you land a supernatural blow against them when you would have crit'ed. Replace your extra damage dice with a d6. So my scythe (at like 2d4+8) became 2d4+32+6d6 (since you replace each extra "crit" die with a d6).

It seems the wording of spell was aimed at big-weapon wielders (who would be using great axes, or other weapons were replace a die with a d6 was a downgrade). But having a character focus on a weapon with a small damage die made it quite nice.
 

open fraction abilities... Hivemind... possessing an awakened tree and waiting a few centuries to die, then coming back one HD lower as your 300HD great oak... 10 armed Iaijitsu master that did a LOT of damage... ohhh yeah
 


Cheater of Mystra FTW. Take the Initiate of Mystra and couple it with Divine Metamagic (persistant) and a couple Nightsticks to get a persistant antimagic field that doesn't affect your buffs. Mix in Dweomerkeeper and you get more spells to cast spontaneously and Miracle/Gate/(Limited) Wish 4/day as a SLA-meaning no XP component.

Of course, you could go really munchy and play a kobold psion 12. I forget which powers are used, but two reptilian creatures can go nuts and make an infinite loop that makes Festering Anger/Cancer Mage look weak.

Oh c'mon--you're totally missing the 3e Weapon Master class to bring it down to 6-20, followed by Favored Critical to bring the crit range to 3-20 if you were fighting your favoured enemy. Then all you have to do is make sure to hit :D Of course, it was probably cheaper to do this with a Mercurial Greatsword and get 15-20/x4 instead of 3-20/x2.
 

Psychic warrior/ Order of the bow initiate. Only one attck every round, but loads of damage thanks to improved psionic shot and feat that allows to become psionically fokused with a move action.
