What would you guard a gate to the Abyss with?

First, encase it in a sapphire, then build a city of paladins around it whose entire purpose is to guard the gate...

If you are just a paladin yourself, its hard. If you have other party members, like a wizard, its easy.
Build a 'dungeon' around said portal. The traps should simply be millions of cheap, permanent magical traps. (Explosive runes, Sepia Sigils, Symbols of Insanity/fear/weakness/death, Glyphs of Warding, etc) Build this 'dungeon' as a one-room thing, fill it with the nastiest magic traps you can. (Permanent Invisible Prismatic Spheres around the Portal work wonders) And further complicate things with mechanical traps like poison gas, maybe a Golem of a nasty nature.
Now, after all that is done, build a huge thick stone wall/dome/barrier that completely encases it. Now encase that in lead. Now put more stone over it. Now get ye olde wizard to move earth/stoneshape/etc to hide the dungeon inside a mountain. I didn't say anything about an entrance, so no pesky adventurer is going to poke their noses in, Lead blocks divinations, so no one will know it's there should they look. And if they do know where it is, if the try and teleport in, even if they don't mis-target, the traps should catch them unaware.

I have to admit, this DOES one-up me. I bow to you.

Humbly Acknowledges Recognition:)

I just figured if you opened up the possibility why limit it to sending them back where they came from? If I were a fiend what would be one place a Gate could lead to that would be the most frightening and ran with it.

Sorry to sidetrack, but I HAD to respond to this. Sure, let 'em play it. And let 'em be surprised when, due to the properties of Neutronium, they end up suffering an effective eternal Imprisonment spell in the very first round of play. :]

Bind the gate to a sapphire, build the sapphire into your throne, and recruit an army of paladins called the Sapphire Guard to ostensibly guard you - but in reality, they're guarding the sapphire. Just no one knows that but a few high-level paladins sworn to secrecy.

Simple. Absolutely nothing could ever go wrong with this plan.

But yeah, Jeremy's right - you don't need to bind an outsider to guard it. A justice archon will do it for free, no bindings necessary. Many other celestials will feel the same.

Build a huge tomb-like dungeon complex. Fill the dungeon with dangerous traps, constructs and undead guardians. The complex should be multi-leveled with heavily locked and barred doors between the levels so that if one level is breached, the invaders have to go through all the challenges and then get through another entrance and start all over again. Leave a few openings in the first level so that local wildlife and demihumans can get in and establish lairs and defenses of their own. Use as much magic as feasible on the lower levels with permanent spells like mentioned above. Make the place a deathtrap.

Take your deathtrap and place it half a mile away from the gate. Surround the gate with a permanent forcecage, surround that with a few feet of adamantine, surround that with lead, then put the whole mess under a pile of solid rock suitable for the terrain. Include some nice nooks and crannies for things like rabbits and birds to take shelter in. Add dirt and grass and whatnot for camoflauge.

Now if you're an adventurer or evil overlord, where are you going to look for the Gate of Unhappy Badness? The huge elaborate dungeon where gobs of danger, experience and possibly treasure awaits? Or that big rock covered in moss where the bunnies live?

Does it just lead to the Abyss in general or to specific spot/location/layer of the Abyss?

That might help me to figure things out...
