Laptop Backpack:
Laptop - which I use to run DnD4e Combat Manager (it tracks all hit points, conditions, powers, etc.) I keep character wish lists, upcoming encounters, various notes, and a few other things on for gaming.
Folder with notes and DM screen if I need it.

My kit is a little unusual because I use a laptop and projector instead of a DM screen and battlemat. I do still have a defined and portable kit, though.
In one hand, I carry the mounting rig for the projector. Base board, pipe flange, 5-foot pipe, elbow joint, short pipe, flange, top board, projector mount, angle-adjusting screws.
In the other hand, I pull a roll-aboard suitcase that contains the following:

- 30-pound sandbag (for holding the mounting rig in place on the floor)
- Digital projector
- Laptop
- Projector power cord
- Laptop power cord
- VGA cable to connect the projector to the laptop
- Extension cord
- Three-way splitter to plug into the end of the extension cord
- Mouse
- Projector remote control
- White vinyl mat (to lay on the table to project the image onto)
- Pens
- Rules Compendium
- A tin of dice
- A handful of minis to use for PCs for players who haven't brought their own
- Bonus Point tokens (to hand to players when they do cool stuff, which can be cashed in to add or subtract 1 from a die roll)
- Some plastic gems to hand out for use as action points
