What's missing from the catalog?

Lost City of Gaxmoor was playtested with an old school-type PC group of 8 or more 1st level PCs, I remember EGG said, and assumes several PC deaths per encounter in the early bits (all EL4-6+). For a more standard 4-PC, few-deaths game I'd recommend it for level 5 at the start, maybe a strong 4th. Encounters go up to EL 20+; with one CR 20 villain my PC group still hasn't defeated after nearly 2 years...

I don't think the planes are cleared for the d20 license yet. Anyone know if they are?

You can't use certain names that are WotC property (such as "The Nine Hells of Baator"), but you can certainly have an adventure that takes place in Hell).

What do you look for or need that no-one makes, or no-one has made sufficiently well?

Have there been any low-level mega adventures for 3.x? Something that takes characters from 1st to 7th or 10th level? I can't think of any.

Looking for a new project ? How about "Corridors of Power: The Book of Politics" - a companion volume to Fields of Blood that offers variants of FoB's Guilds, usable as non-landed power bases ?

Seriously, FoB is a very good stab at the politics, and it encompasses a lot more than many others. However, the picture is incomplete while the Guilds (Thieves, Wizards, Churches, Druid circles) remain only passive tools for the land-based ruler.
