There was one massive roll of about 38...

Then again, we tended to stop games pretty close to 10th level; after that, we kinda lost interest.

Well, why not just ask what the highest modifier anyone's ever had on a roll?

There was a character like that in my game.

Ranger
Special divine boost to a few skills (+3)
Luckstone +1
Skill Focus +3
Those +2/+2 feats
Wisdom increasing item +2
... probably other stuff, can't recall.

He would regularly get above 40 on his Survival and Spot checks.

My pixie warmage in WLD had a +54 Hide skill when he was invisible and not moving. I believe I got into the low 70s once or twice, but can't remember if I ever rolled a 20 on one of his hide checks.

Same situation here but it wasn't a purple worm. The PC was a buffed barbarian/cleric in rage with Divine Power or Righteous Might and rolled nearly maximum damage with all three d10... added together to more than 80 points of damage... added to the grapple check.
 

not so impressive as some of the other posters but I've hit numbers somewhere between 40 and 50 with the following:

halfling thief/fighter/peerless archer (12th level), maxed out hide (and move silently), high dex (20), racial boni, etc, but no magic enhancements

459 as a single hit against a Giant in Rolemaster. The joys of rolling d100s for add-and-roll again if you get 95-00. Three rolls in the high nineties plus a decent OB means on amazing, memorable end to the beastie :)

My 11th level druid gets +24 on her survival checks, and has got checks in the 40's regularly. My 9th level monk has +32 on jump checks and has got into the 50+ range once.
