In my human-dominated campaign world elves & dwarves are dying races while gnomes & halflings, if they exist at all, are so rare as to be classed with pixies and other 'faerie'. So I encourage players to play humans - a lot easier in 3e than previously, one thing I love about 3e. There are half-elf barbarians and I might allow a grugach (wild elf) barbarian, depending on where the game was set. I certainly wouldn't allow an elf barbarian just because they 'ran off into the wilderness' - Ranger, yes.
IMO, PC's are supposed to defy conventions to a certain extent. The DM should work to maintain consistency of the game world and keep everything reasonable, but you should also work to encourage the creativity of the players instead of limiting it. Using cultural reasons to place an all out ban on certain race/class combos is a sub-optimal choice, when you could just tell your players the way most of the world works and if they want to be an elven barbarian, tell them to bring a good background story. And definitely make sure you enforce the consequences of their choices

I don't totally disagree, but my Conan-esque game world works best with human PCs, non-human PCs are best kept as a small minority. I do disagree strongly with any proposal that the GM should have to accept elf barbarian or dwarf wizard PCs just because the players want to play them. Obviously if no agreement can be reached, the solution is to have a different GM.
