When fighting an incorporeal creature...

...the description is:

Can be harmed only by other incorporeal creatures, +1 or better magic weapons, or magic, with a 50% chance to ignore any damage from a corporeal source. Can pass through solid objects at will, and own attacks pass through armor. Always moves silently.

Does this mean that if you are not using a +1 or better weapon, you have a 50% chance to miss, or does it mean that you have a 50% chance to miss even if you are using a +1 or better weapon.

If you are not using a +1 or better weapon, you can't hurt an incorporeal creature, period. If you are using a +1 or better weapon, you are subject to a 50% miss chance on each hit, unless the weapon has the ghost touch enchantment.
 

Non-magical weapons can't affect incorporeal creatures.

Using a magic weapon or spell you have a 50% chance

Using a ghost touch weapon or a Force spell effect (such as magic missile) you will always hit (assuming you make the AC)
