Does anyone know where to find The Everyone Everywhere List by Erik James Olsrud?

Google, RpgNOW and Amazon have failed me.

I have a copy of the first, pre-d20, version (both printings are identical with regard to content, so far as I know). I've cut it apart with an exacto (the staples fell out long ago). It's just a list of ethnic names, really. In many ways, you'd be just as well off randomly picking a first and last name out of a larger, metro area, phone book.

I haven't seen it in stores for years. Noble Knight has a copy, but that's pretty pricey for a stapled ~20 page list of names.
