Where do find the Multi-headed template?

WotC had a template for creatures with multiple heads. Anyone know where it was published? On their website, perhaps?

I'm expanding on that idea for the bodak template that I mentioned last week. Sort of a bodak hydra

Tome of Horrors or Creature Collection II (I'm pretty sure it is ToH) has one. I just bought both and don't have them nearby or I'd let you know for sure.
