Where is this creature from?

It is in the 2nd Edition MM - at least, it's in the printing I own (shiny black hardbound). Page 207.

It's also one of the "Four Waterdhavian Creatures" from the initial quest in Neverwinter Nights.

Which was also buggy. Obviously it's hard to simulate the intellect devourer on a computer.

I'm fairly certain it was in the 2ed monster manual as well.
IIRC, in 2e the intellect devourer first appeared in the Psionics Handbook. When they abandoned the Monstrous Compendium (looseleaf binder as opposed to hardback), it got included in the new MM as well.

The book was called "The Complete Psionics Handbook" to be precise.

The Intellect Devourer was first printed in the 1st Ed:

And most recently reprinted for 3.5 in the Expanded Psionics Handbook:

To be honest, the Intellect devourer bug was one of the oldest in NWN. It was discovered right about the time the game was released, and didn't really have anything to do with the critter itself. I thought the designers nailed the idea of the bugger pretty darned well.

Admitting I might be reading too much into your comment... :)

Wow, they sure weaken them in the 3rd ed. In the 1st, they were almost impossible to kill. Magic weapons +3 or better did one point of damage, fire did nothing (but they ran from it anyways), lightning did 1 point per die, death spell had a 25% chance of working and only power word: kill would do the trick without a chance of failure.

I hate the buggers because of that. Even the aumorovorax, which had 12 HD for a small creature could be killed without knowing what it was, but not these guys.
