IIRC, Night Dragons first appeared in GAZ3 Principalities of Glantri. As far as release dates, I'd have to look them up.

So many. I focused on mods that have received no 3e treatment thus far (most of them:()

Against the Giants, Caves of Chaos, D1-3, Desert of Desolation series (which never got a 'return to' module in 2e like many classics did), Slavelords, and the Forgotten Temple of Tharizdun.

One that I would vote for that was not listed is the Desert Nomads series.
 

For what it's worth, 0one Blueprints has a beautiful PDF map that greatly expands and replaces the B2 Caves of Chaos. The size of it is daunting to me, but I'm strongly considering using it instead of their Hill of Five Dungeons for my super-sized kobold lair in my campaign.

Night Dragons first appeared in Dragon #153 or 154 (I think it was 154), and were later given stats in Dragon #163. Both articles appeared later than CM1: Test of the Warlords, and I believe Gaz3 came out before them as well.
 

It sounds like you've nailed down the Ur-Template, then! Now if someone ran a 1E bard through Test of the Warlords, you'd have the first prestige class facing off against the first template ...

Oh my gosh, the preview in the latest Dungeon rocked!
Every adventure path should be previewed like this before it starts.

Oh and way to go on keeping Nick Logue around...I've gamed with him. I like his writing as well.
