Things I loath and house rule for the sake of "reality" or more accurately the continued and necessary suspension of disbelief and versimilitude:
No one falls 200ft and lives...the damage system I have created makes such a survival nearly miraculous.
1. Fall into lava and you die.
2. Fixed HPs (using WP/VPs instead). No one fights until passing out without consequences.
3. The world doesn't level with the PCs, certain regions contain greater or lesser threat levels, go to a region known to be inhabited by malevolent giants, you die.
4. LOW level magic is sold ie some basic scrolls, potions and weapons (+1-+3 enchantment) and not in shops, but from mages or temples. More powerful items must be discovered or comissioned. Only a black market dealer can make certain particularly dangerous items because most sensible civilized lands either ban items that can be used to topple the government or only allow their use by those santioned to do so.
5. Switched to the silver standard with copper being the most common coins. Gold is not found in great abundance save in the safekeeping of the truly wealthy, in the form of jewelry or in an very ancient and powerful monster's lairs (those powerful enough and those that have the time to collect large quantities of gold).
6. The gods have no stats.
7. No alignment, people do what they do for complex reasons. Certain beings are still "evil" by any reasonable standard but there is no way to know unless you see them act in a given manner.
8. Monsters only carry treasure if they would value it personally or use it personally (in the case of magic items and weapons).
This is some of the changes I have made for "realism" and "fun" and my players are happy. Before I insituted the fall into lava you die rule, I once had a PC fall into lava and survive. The player, my brother said, "No he's dead. That's ridiculous and I don't want to continue to play a guy that I can't take seriously." I had him die from the lava and learned a valuable lesson that day. For some folks, versimilitude matters more than official rules and make for greater fun overall.
