I suspect that it would logically reference some of the content of the Book of Vile Darkness, particularly ways to counter vile damage and some of the spells. Thus, WoTC might be labelling it as mature because of some of the earlier material.
Gez, I agree that an Underdark book should be about a lot more than drow. The Svirfneblin, the deep dwarves (neutral but sometimes confused with the duergar), and other races should be examined in detail. A good map of where some Underdark locations are in relation to the surface would be an added bonus.
The ecology of the Underdark alone is worth a section, as the question is what is at the base of the food pyramid. (Don't say adventurers.)
