What if you move past an ogre with combat reflexes? There are 4 space you move from that he threatens, does he get one attack per space, of if you move through all 4, he can only make one attack?

Leaving more than one threatened space during a turn counts as a single provocation.

He gets one attack because leaving threatened square 1, leaving threatened square 2, leaving threatened square 3, and leaving threatened square 4 all count as the same provocation.

If you had a really, really high speed, you could run in circles around an ogre, run past him three or four times, and still only provoke a single AoO.
