Looking towards ability scores, her original stats are listed as:
S: 20 (+3/+8) I: 25 W: 23 D: 18 C: 21 CH: -1

Obviously, the -1 Charimsa needs to go. Since Charisma has evolved into much more than looks, she should probably have a rather high Charimsa, but perhaps suffer a penalty on all Charisma-based skill checks due to her hideousness.

Her other ability scores are still a bit on the low side for comparable unique outsiders of similar power. For comparison:

Geryon: Str 36, Dex 19, Con 35, Int 26, Wis 24, Cha 23
The Hag Countess: Str 31, Dex 26, Con 24, Int 28, Wis 31, Cha 28
Yeenoghu (BoVD version): Str 37, Dex 28, Con 32, Int 26, Wis 25, Cha 25
Kharash, The Stalker: Str 37, Dex 32, Con 29, Int 26, Wis 28, Cha 25
Barachiel, The Messenger: Str 27, Dex 34, Con 27, Int 27, Wis 20, Cha 29

hag countess makes for a good superficial comparison, but i would take the others into account as well.

Abilities: Str 19, Dex 12, Con 18, Int 11, Wis 15, Cha 12

I'm inclided to give her a decent CHA score and just use the 2nd method of a Fear aura to explain the disgust.

Looking at some of the differences:

Hit Dice: The 2E version translates to 33 HD in 3E
Speed: 20 feet vs. 30 feet (I think 30 feet is more appropriate)
Her natural attacks are much more potent.

Here's my suggestion
STR, 27

which would make her attribute spread better than Geryon and Barachiel, but not better than the hag countess, Yeenoghu, or Kharash

Agreed on decent Cha score, like the Hag Countess.

I don't believe Iggwilv has been statted, but I could be wrong.
