I've been looking for initiative cards and not found what I wanted, so I threw these together. What I wanted specifically was a card with the basic info, plus a place to track HP, action points, second wind, and effects. There's a character card and a creature card; the creature card has space to track HP for four creatures. The empty lines in the middle are for attacks, notes, etc.
The cards are the standard 3.5" x 2.5" MTG size. The PDF file has six character cards and six creature cards. The document is a PDF form, you can fill it out before printing or just print blanks.
When you go to print, in the "Print Range" section, choose the Pages button and pick the specific pages you've filled out; 1-6 are character, 7-12 are creature. Then in Page Scaling choose "Multiple pages per sheet" and in Pages per sheet choose "6", and select the Auto-Rotate Pages checkbox. This will print six cards per page at the correct size.
