A medium sized creature attacking a large creature would provoke an AoO when entering a threatened square?

Perhaps this is a silly question, but I can't find any rule that says entering into a threatened square provokes an attack of oppotunity. Does it? If I do a regular move into a threatened square and then attack using my standard action, does the move provoke an AoO? What if I'm charging?

I would lalow it as well, but the group I play with take turns DM'ing. For this reason they don't allow anything that isn't in the rules. I've been using parry for as long as I can remember. i just assumed it was in the DMG.
