Definetely the Temple of Elemental Evil. I remember that the check for random encounter is made at each 10 minutes! So it's 48 checks for a night of sleep! It is simply impossible to sleep in there.

I've made such a program although it doesn't do all the things you want, it does let you creates your map and run them on the computer. You can download it here I'm also working on a new version that let you resolve combat turn by turn. I've also included a lot more icons to create your maps.
