Did anyone ever name the kingdom (or kingdoms) depicted on this classic map?

Outdoor Survival, by Avalon Hill.

It's tied into D&D because in OD&D, it was mentioned to use its rules for outdoor adventures. (For more info, read here and here).

OD&D, Vol. 1, "Recommended Equipment" lists (1) the D&D game, (2) Outdoor Survival, (3) Dice, and (4) Chainmail. Note how it comes before the dice or Chainmail that actually has the primary combat rules.

OD&D, Vol. 3 says, "All terrain penalties are as stated in OUTDOOR SURVIVAL... A lost party must move in the direction indicated by the die roll (1-6, as shown in the OUTDOOR SURVIVAL rules and on that board)...", etc., etc.

Note that it's not too hard to get. On eBay past auctions could have gotten it for slightly over $10 with shipping & handling.
