Counting ain't it's strong suit. How about asking for a duo of heads?
Tried it. Same thing.
Gave me five at one point. I just posted the ones with two.
For me, it's three EVERY time. I tried "two-headed", "with two heads", "2 heads", "duo of heads", "double-headed", etc.

I was curious to see if it could differentiate between a low-level and a high-level adventurer. Doesn't seem like it.

Art style was supposed to be the same, but if you ignore that, there's no real significant difference. Ah well.

So, when DALL-E starts to frustrate me by ignoring clear language and starts adding stuff I am not asking for... like arms and claws... I go back to basics and see if I can get something in isolation to work. I try and remove any complexity so I can perfect the part of the prompt that is not working as part of a greater whole.

I started with it just making a tentacle on a white background.

I then try to use this success to take the next step and I ask it, "Now draw a tentacle attached at the shoulder joint on a humanoid torso. Maintain the white background." I am not being super specific yet, I just want to see what it does with the simplest command. It gives me this...

Not what I wanted. At least it is on the arm... sigh.

Ok, I ask it to refine, "This is wrong. The single tentacle should act as a complete right side appendage on the torso." That generates this... partial success...

GENERATED PROMPT: A realistic depiction of a humanoid torso where the right side appendage is replaced entirely by a single tentacle, set against a plain white background. The torso is detailed and lifelike, while the tentacle seamlessly integrates as the right appendage, showing natural curves and textures in a realistic art style.

Unfortunately I started getting errors when I tried to keep the torso clean and lengthen the tentacle. I will try again a little later.

I think the three-head thing might come from the fact that the source material for two-headed baboons is probably roughly evenly split between ones facing left, ones facing right, and ones with one face directly toward the 'viewer.' These sorts of systems are basically autocomplete but for pixel arrangements, and so when it tries to draw this, it knows it needs more than one face but can't figure out whether the second face should go on the right or the left, so it basically does both.

Well I finally got a tentacle appendage on a human torso, it can be done... but Greyhawk gods have mercy it is like pulling teeth...

GENERATED PROMPT: A realistic and detailed depiction of a humanoid torso with the right side appendage replaced entirely by a single tentacle. The torso should be detailed and lifelike, showcasing natural skin tones, muscle definition, and anatomical accuracy. The tentacle, replacing the right arm, should integrate seamlessly with the torso, exhibiting natural curves and a smooth texture, consistent with a realistic art style. The background is plain white to highlight the contrast between the humanoid and tentacular features. The focus is on realism, with fine details and textures to create a striking and believable image.
