This is a little thing I whipped up — a 4-page A5 booklet-fold spell book, with spaces to record spell level, name, casting time and components. Print on both sides of an A4 sheet (or letter, as long as you have "fit to page" turned on), and fold in half to create the booklet. If you need more pages, just print multiple times and saddle-staple.

It's pretty generic, and would suit pretty much any edition of D&D.
