    I prefer the word "flavour" to fluff, as, IMHO, it more readily captures the essence of the concept in contrast to "crunch". "Fluff" seems too dismissive for me.

    This has changed in 3.5; you are allowed to make one AoO per *opportunity*, though most combatants still only have one attack per round. Combat Reflexes allows you to make an addtional attack for each additional opportunity given by an opponent, up to your Dex mod. (PHB p92.)

    I've only known one person to have purchased it; a good friend of mine. He was disappointed as soon as he opened it: light paper, nonsensical maps, not set to 1" grids, and no text/booklet of tips/cool stuff. All the maps can be found on the WotC site for free... just print the ones you need...
