You are missing/misunderstanding one concept in your concern: The Underdark isn't separate from The World, like the Elemental Chaos or the Astral Sea are... it's just what lies beneath the ground of The World. It's part of the The World. (And thusly, a reflection of The World should/could...

Here's another vote for the versatility of M&M2E. While it eventually morphed into a Golden Age Supers campaign, I started my campaign as a straight up WWII GI combat type game. The PCs were PL 4, and we could easily have gone awhile like that, but I had different plans for them.

I've had a pretty good campaign using Mutants & Masterminds to run a fairly gritty World War II superhero game... I'd suggest that system for this as well.
