    Especially because a large part of the poison's cost is (IMO) meant to reflect the difficulty in getting the ingredients.     I think it would be fun to keep that "roll d3" for number of hits. You could make each head deal 1d4. That way, average damage is 2d4, which is not particulary broken. Plus, you get cool suspense when you crit and stuff.

    Also, the PHB states that a large creature making an attack can "use any square it occupies for purposes of determining cover." So if you are large and your ally is in front of you, your enemy may not get cover depending on which square he's in.

    I would also have to say combat casting. The feat is pretty much obsolete with skill focus (concentration). Hmm... I'll take 1 less on casting defensively, which after 10th level I can pretty much do for free anyway, and I get +3 on all other concentration checks.

    Min/maxing a human barbarian

    I'd switch his con and dex scores; barbs need a lot of hps to survive. And when he rages, he'll be takin -2 to ac anyway. Also, switch int and cha; extra skill points are useful, being a charasmatic barbarian isn't (unless you want to do some kind of leader, or a lot of rp, but you said min/max).
