    In regards to the OP, you can't do this because you must have line of effect: Actually, you don't need line of sight for most spells. You can simply declare the origin of the spell with a relative position (such as "100 feet to the north.") If it turns out that there was a wall in the way...

    Increasing the damage of bows in the way proposed is statistically the same as giving them all +1 to damage. Except the heavy crossbow, which gets a whopping 1.5 damage. I doubt it is overpowered, but I don't particularly think its necessary, either. :)

    And that list doesn't include all the creatures in the monster manual which have LA's. (Notice that it doesn't even contain the regular Orc.)

    Being of large size is, in and of itself, a huge deal. You get natural reach, increased damage, and a nice bonus on disarm/grapple/trip checks and the like. It's pretty much worth LA +1 its own, despite the penalties to hit and to AC. And the +6/+2 to Str/Con is probably better than +4/+4...

    Maybe it would work with an LA applied to the whole concept. Because either you care about balance, or you don't. But either way a triple gestalt just flat out isn't balanced. ^_^

    White Raven Tactics inherently leads to infinite loops. All you need is two PC's with it and it springs right out. It's just a broken maneuver that should have been caught in development. (anything that messes with initiative should raise redflags when playtesting.)

    Tome of Battle Monk class?

    I think it is called "Superior Unarmed Strike." It gives you a wussier version of the monks unarmed damage progression. (Tops out with 2d6 at 20th level, I think.) Or, if you're already a monk, it grants you unarmed damage of a monk 4 levels higher.

    My gut feeling is that this is too weak for an LA +2, and could probably get away with being an LA +1. Compare it to the Goliath. Goliaths get +4 Str, +2 Con, and powerful build. I don't think swapping powerful build for +2 natural armor, slightly better stats, and weak energy resistance...

    Seconded. Simply make the system smaller, and you won't have a problem. (And the only reason I can think of to make a system that huge in the first place would be to drastically restrict travel, which obviously isn't your intent. :P)

    In D&D, the long sword is a one-handed weapon. A one-handed weapon is considered one size category smaller than the being it's made for. So a medium sized long sword is a Small object, and a large sized long sword is a Medium object. So monkey grip lets her use a weapon that's somewhere...

    Shatter at will is probably better than explosives. If you have anything made of adamantine, it'll let the barbarian knock down the pillars really fast.

    On the one hand, I don't particularly like some of the answers (such as how wizards use metamagic rods.)     Actually, the crusader pretty much never runs out of maneuvers. After playing using the ToB, I think that, even if it's not unbalanced, they could have made the maneuvers lower powered. It still would have been a lot of fun to play with.
