It may not have become a lich willingly. In the world I made, as long as something has a strong enough soul and a suitable receptacle nearby to act as a phlactery, it can become a lich or just an undead if no phlactery is nearby.

My campaign setting is a world I created. It has a 25 abominations/monstrosities that are fairly strong as well as a few high level adventurers that could have brought it near death. It is a world rife with undeath

Just for clarification I am using 5e but I'm not opposed to translating in things from older editions
