Any setting can be harsh and unforgiving if run properly... that is the way any PC should live in my mind. There are sections on just about any setting that could be run this way. Ravenloft and Scarred Lands are the best in my opinion, depending on whether you want horror or just blood and grit.

Re: PPP We do even less than that... we don't even talk about it. Pick it up, stick it somewhere, keep playing. We don't get to play often enough to be adding up the weight of a bunch of loot. ;)
