    Because his injury in D&D would not be represented by loss of HP, it wasn't caused by an attack, and even if it was so, there are no rules in D&D for disabling injuries unless it's from a specific ability or power. The close to that condition would be stat loss, or some effect of a specific disease.

    You only really need "HP as moral" when you have non-magical healing abilites that work by "increasing morale", like Warlord powers from 4E, or if you just want to. If a nick of a sword deals Damage, some of that damage may be loss of morale, even if a very little loss of morale.

    Ok. Well, I do interpret that all damage is caused exclusively by some physical harm, but not all damage is a physical harm. I think the combat system feels a little more "realistic" that way, and also easier to handle.

    You won't have Schrodinger's HP at all with shouty-healings and full overnight rests, or any other non-magical healing, if you disassociate damage from wound. HP loss represents damage, not wounds. HP gain represents the mitigation of damage, not the undoing of injuries.
